Motiv Rehabilitation Inc. offers home visits to clients looking to focus on their functional and rehabilitative needs. Motiv will focus on ACTIVE Rehabilitation and Exercise Therapy to help clients return to independent living, return to work and/or daily living programs.

For almost 30 years I have helped many clients and their families work through their rehabilitative process after a traumatic injury such as stroke, medical diagnosis, traumatic brain injury or soft tissue injury and continue to work to support and guide clients through the recovery process in a timely and efficient manner.

In my experience, it takes a solid and well informed care team and proactive nature to expedite appropriate recovery outcomes.

Each consultation is confidential and geared towards the best interests of each client. My goal for clients is independent recovery. At times and with appropriate care plans in place, each client can succeed well and have a very high quality of life moving forward.

Case management, Active Rehabilitation, Personal Training and home rehabilitation are areas I cater to.

Christin Chernoff has returned as a resident of Tsawwassen having graduated from South Delta Senior Secondary in 1990, graduated from SFU in Rehabilitation Management (2010), is a Certified Physiotherapy and Occupational Therapy Assistant (1994) as well as a current registered BCRPA Certified Weight Trainer and Personal Trainer (1995). Current PTA member of the Canadian Physiotherapy Association. Specialty courses in Senior wellness, injury prevention and Junior Fitness Programming for youth is ongoing.

Experience in tertiary care, sub acute and complex care, outpatient clinics, community and in facility rehabilitation have given me the experience required to work with this very special population of people.

Neurorehabilitation such as recovery and management from Stroke, Multiple Sclerosis, Parkinson's disease, fall prevention, spinal cord injury rehabilitation and traumatic brain injury rehabilitation are specialty areas I continue to work in.

Each rehabilitation program is assessed and approved by a certified member of the Canadian Physiotherapy Association, Canadian Association of Occupational Therapists, Physician or Specialist in the field of Rehabilitation. Physical and functional training programs may require Physician consent and approval.
